(A) Any person who engages in the practice of massage, as defined in § 113.01 of this chapter, shall file an application for a permit as a masseur or masseuse.
(B) The application shall be filed with the Village Clerk upon a form provided by the Clerk.
(C) The applicant shall pay to the Clerk a non-refundable filing fee of $25 for the original application and $25 for each renewal application. The Clerk shall issue a receipt which shall be attached to the application filed with the village.
