§ 112.01 APPLICATION.
   (A)   Applications for all licenses and permits required by this code or any other applicable ordinance of the village shall be made in writing to the Village Clerk, in the absence of provisions to the contrary.
      (1)   Each application shall state the name of the applicant, the permit or license desired, the location to be used, if any, the time covered and the fee to be paid.
      (2)   Each application shall contain additional information as may be needed for the proper guidance of village officials in the issuing of the permit or license applied for.
   (B)   Applications for business licenses, whether retail or otherwise, shall state the village code number shown on the applicant’s certificate of registration issued by the Department of Revenue, pursuant to ILCS Ch. 35, Act 120, § 2a.
      (1)   The Village Clerk shall not issue any business license, whether retail or otherwise, unless the village code number stated into the applicant’s application is the same number assigned to the village by the Department of Revenue.
      (2)   Each application shall contain additional information as may be needed for the proper guidance of the village officials in issuing the license applied for.
