A. Bond Required: It shall be the contractor's responsibility to post a two thousand dollar ($2,000.00) bond per project with the Town for a two (2) year period covering workmanship and materials, or may have the option to post a ten thousand dollar ($10,000.00) annual bond for three (3) years starting from January 1 of each year. The original bond must accompany the building permit.
B. Permit Required: A road cut permit from the Town is required for all work done within the Town right-of-way or a permit from State Department of Transportation for work performed within the State right-of-way.
C. Fees: All initial fees must be paid with the building permit fees. Additional inspection fees may be billed to the contractor at a later date.
D. State License: Contractors must possess the appropriate State contractors license and bond for the type of work being performed.
E. Standards; Notice: All work shall conform to the State, AWWA, Town Engineer and Inspector standards for material and workmanship. It shall be the contractor's responsibility to notify the Town five (5) days prior before work will commence.
F. Utility Line Installations: The Town utility line installation shall comply with Utah DEQ guidelines, AWWA standards and must be approved by the Town Engineer.
1. All PVC pipe shall meet AWWA Standards C-900 with dimension.
2. Ratio (DR) 18.
3. Service taps three-fourths inch (3/4") on PVC main shall be made through a bronze corporation stop and bronze double strap tapping saddle. Service three-fourths inch (3/4") may be direct tapped into case or ductile iron mains.
4. Meter boxes for three-fourths inch (3/4") services shall be round concrete or plastic with an eighteen inch (18") inside diameter. Lids shall be case iron with a pentagon locking nut and provide for a minimum twelve inch (12") opening.
5. Meter setters for three-fourths inch (3/4") shall be of copper and bronze manufacture with a lockable angle meter valve on the inlet side. A dual check valve that is repairable without removing the meter is required on the outlet side.
6. All pipes shall be bedded in sand or other suitable granular materials with no rocks larger than three-fourths inches (3/4"). No organic or other deleterious material will be allowed in the bedding.
7. Backfill material in the Town right-of-way shall be granular with no rocks larger than two inches (2"). It shall be free from organic material, clay, dirt or other deleterious material. Backfill material will be placed in twelve inch (12") lifts and compacted to ninety five percent (95%) density. Flowable fill may also be used for backfill material and poured in one (1) continuous lift after pipe is suitably bedded in sand.
G. Town Surface Restoration:
1. It shall be the contractor's responsibility to replace/restore all surface improvements to equal or better condition, in all respects that the existing improvements removed.
2. Road base shall be placed to a minimum depth of eight inches (8") and compacted to a density of ninety five percent (95%).
3. All existing asphalt shall be a saw cut back to a minimum of six inches (6") beyond the limits of any trench or cave in. All edges will be tack coated with MC-70 oil before patching. Asphalt will be placed to a minimum depth of three inches (3") and compacted to a density of ninety five percent (95%).
H. Inspection And Other Fees For Contractor Installed Services: Inspection shall be by the Town Engineer or designee. The Town will not perform new installations. A contractor approved by Town Engineer will install services. All costs will be the responsibility of the owner.
1. Inspection fees shall be in such amounts as established by the Town Council.
2. The contractor must procure a permit from the State Department of Transportation for excavations on any State road and present it to the Town prior to beginning work.
3. No Town employees will be allowed to enter excavations that do not meet OSHA safety requirements.
