Applications for excavation permits shall be made to the department and shall describe the location of the intended excavation, including boring, the size and scope of the excavation, the purpose for the excavation, and the person or business entity doing the actual excavation work, and the name of the person or business entity for whom or for which the work is being done. The application shall contain an agreement that must be signed by the applicant stating that the applicant and all contractors will comply with all ordinances and laws relating to the work to be done.
