3-3-4: APPLICATIONS FOR LICENSES:
   A.   All applications for licenses shall be verified in writing on the form provided by the Local Liquor Control Commissioner signed by the applicant if an individual or by an officer or authorized agent accompanied by a certified copy of a resolution authorizing application if a corporation and if a partnership by all of the partners.
   B.   All applications for renewal of license shall be made on or before thirty (30) days prior to the date of expiration thereof.
   C.   Each application shall be accompanied by money order, cash or cashier's or certified check in the amount of the fee for the license applied for, which fee shall be returned to the applicant if such application is denied.
   D.   All applications shall contain the following:
      1.   The applicant's name, address, citizenship, date of birth, social security number, position and percentage of ownership in the applicant business; and the name, address, citizenship, date of birth, social security number, position and percentage of ownership in the applicant business of every sole owner, partner, corporate officer, director, manager and any person who owns five percent (5%) or more of the shares of the applicant business entity or parent corporation(s) of the applicant business entity.
      2.   In case of a partnership, the date of formation of the partnership; if applicable, the date of filing and a copy of the "assumed name" registration certificate of the business filed with the County Clerk; in the case of an Illinois corporation, the date of incorporation and a copy of its articles of incorporation and if a foreign corporation, a copy of its certificate of authority to transact business in this State.
      3.   The amount of goods, wares and merchandise on hand at the time of the application.
      4.   If applicant is applying for a license for off-premises consumption only (see subsection 3-3-8A2), a diagram of the space to be used for liquor display is to accompany said application indicating that such display is in a highly visible and controllable location. Applicants for on-premises consumption must provide proof of liability, including "dram shop" insurance.
      5.   A statement whether the applicant has made application for any liquor license other than for the subject premises and the disposition thereof.
      6.   A statement that the applicant has never been convicted of a felony and is not disqualified to receive a license by reason of any matter or thing contained in the provisions of this Chapter or rules and regulations of the Illinois Compiled Statutes or rules and regulations promulgated thereunder.
      7.   A statement whether any previous license has ever been suspended or revoked.
      8.   A statement that the applicant will conduct his business in accordance with applicable Federal and State laws.
