Application for licenses required by Section 733.02 shall be made to the Fire Chief upon such blanks as shall be prescribed by the Chief and upon approval of the application by the Chief a license shall be issued by the City Manager as provided by Section 733.02. In making the aforesaid application, the applicant shall state his name, whether an individual firm or corporation, the place or places at which the business is conducted or maintained and further information as may be required by the Chief.
