§ 18-132 APPLICATION FOR CONNECTION PERMIT.
   (A)   Every application for a water or sewer connection shall state the name of the owner of the lot, the name of the street on which the lot is situated, the number of the building if there is one on the lot or, if not, a description of the location of the lot, the number and kind of the connections desired, the character of the surface of the abutting street and any other additional information required by the Administrator. Every application shall be signed by the person making the application.
   (B)   Every application for connection shall be accompanied by the following fees pursuant to § 18-108 of this chapter unless such fees have previously been paid or are otherwise inapplicable:
      (1)   Lateral and meter installation charges. When these charges are assessed on an actual cost basis, the applicant shall deposit with the city an amount equal to the estimated cost of lateral and meter installation charges. If the cost of connections is less than the installation deposit, the difference between the deposit and the connection cost shall be refunded to the applicant. If the cost of the connection is greater than the installation deposit, the applicant shall pay to the city an amount equal to the difference between the cost of connection and the installation deposit; and
      (2)   Service application (see § 18-105).
   (C)   When lateral and meter installation charges are collected the service initiation fee provided for in § 18-109 of this chapter is waived.
   (D)   No permit shall be issued for water or sewer connection until a determination is made by the City Engineer that water and sewer service is sufficient including approval of public utility infrastructure improvements are necessary to ensure adequate water and sewer service is available to serve the subject property and required performance bonds or other form of acceptable financial surety has been received by the city and all associated fees have been paid in full by the applicant.
