§ 93.03 APPLICATION AND CASH DEPOSIT.
   Each permit for making the opening shall be confined to a single project and shall be issued by the mayor or his authorized agent. Application shall be made on a form prescribed by the city, giving the exact location of the proposed opening, the kind of paving, the area and depth to be excavated and those other facts as may be provided for. The permit shall be issued only after payment of a permit fee and making deposit sufficient to cover the cost of restoration has been posted with the city clerk, conditioned upon prompt and satisfactory refilling of excavations and restoration of all surfaces disturbed.
   (A)   The following procedure will be followed:
      (1)   The superintendent of public works will set bond amount.
      (2)   Bond will only be accepted in the form of:
         (a)   Cashiers check or
         (b)   Cash, performance or surety bond
      (3)   Bonds will be held by the clerk's office.
      (4)   The superintendent of public works or public works inspector shall inspect the restoration of street openings.
      (5)   In the case of utility companies:
         (a)   All utility permit applications shall be signed by the city official authorizing the work and the permit forwarded and filed in the office of the superintendent of public works.
         (b)   Utilities will not be required to take out a permit to work on existing utility owned electric lines, water lines, gas mains, or other items, such as meter pits, etc. unless in the opinion of the superintendent of public works, the utility has displayed a record of insufficient restoration.
