(A) All body artists and body art establishments shall maintain records of each patron for two years.
(B) The record shall include the following:
(1) Patron’s name;
(2) Patron’s address;
(3) Patron’s age. Age must be verified by two items of identification, one of which must be a valid government issued identification;
(4) Date body art was performed;
(5) Design of the body art;
(6) Location of the body art on the patron’s body;
(7) The name of the body artist who performed the work; and
(8) Parental consent must be maintained, in writing, when performed on any person under 18 years of age, as permitted by law.
