The following is the checklist for engineering plans:
(A) __________________________________________ (Name of Subdivision)
(B) __________________________________________ (Date of Submission)
(C) ___________________________________________ (Due Date of Recommendation - 45 Days)
(D) (NOTE): To properly execute this checklist, the subdivider or his or her engineer shall:
(1) Insert the required information.
(2) Denote compliance with applicable ordinance by placing his or her initials in all spaces where applicable.
(3) Denote those items which the subdivider considers "Not Applicable" to this particular subdivision by the abbreviation, "N.A.".
(a) ________ Plans have been submitted within 12 months of the date of approval by the Village Board of the preliminary plan.
(b) ________ Four copies of engineering plans have been submitted.
(c) ________ Plans conform to § 154.076.
(d) ________ A title sheet is included with each set of plans and includes:
1. ________ Name of subdivision and unit number;
2. ________ Type of work covered;
3. ________ Location map showing relation of area to be improved to existing streets;
4. ________ An index of sheets;
5. ________ A summary of quantities;
6. ________ Name, address, and seal of registered engineer preparing the plans.
7. ________ Date of preparation and revisions, if any, is shown.
(e) ________ Plan and profiles are on Federal Aid Sheets, Plate I or II, or equal.
1. ________ Horizontal scale is not less than 1" to 50';
2. ________ Vertical scale is not less than 1" to 5'.
(f) ________ Cross sections are plotted on Federal Aid Sheets, Plate III.
(g) ________ North direction is shown for each separate plan view.
(h) ________ An adequate number of bench marks are shown with elevations referenced to mean sea level to facilitate checking of elevations.
(i) ________ Delineation is shown of all easements necessary to serve all lots with underground and overhead utilities, and to allow for perpetual maintenance to these facilities.
(j) ________ An application for State Environmental Protection Agency permit for the sanitary sewer extension accompanies the plans.
(k) ________ Sanitary sewer plans and specifications are complete and conform to the standards and requirements of the Codes applicable thereto and denote all of the following: (See Chapter 53)
1. ________ All properties in the subdivision are served and house service connections are provided.
2. ________ The minimum size main is 8" I.D.
3. ________ The plan conforms to the overall municipal plan for any trunk sewers traversing the subdivision.
4. ________ The distance between manholes does not exceed 400'.
5. ________ The invert elevation of each manhole is shown.
6. ________ The grade of each section of sewer is shown by percentage in accordance with accepted engineering practice.
7. ________ Extra strength pipe and extra strength manhole wall construction is specified and shown on the plans and in the estimates of quantities where the depth of installation exceed 8'.
8. ________ Profile of existing and proposed ground surfaces.
9. ________ Risers are shown for individual house service laterals where depths of main exceeds 12'.
10. ________ Pipe joints are of permitted type.
a. ________ 540 pounds in collector streets;
b. ________ 400 pounds in minor and cul-de-sac streets;
c. ________ 335 pounds in rear-lot easements.
(l) ________ An application for State Environmental Protection Agency approval of the water main installation accompanies the plans.
(m) ________ Water distribution plans and specifications are complete and conform to the codes applicable thereto and include all of the following:
1. ________ All properties in the subdivision are served;
2. ________ The minimum size main is 8" I.D.
3. ________ The plan conforms to the municipality's overall plan for any trunk lines which might traverse the subdivision.
4. ________ Valve and hydrant spacing and location conform to the approved preliminary plan.
5. ________ Materials and joint specifications comply with the municipality's standards.
6. ________ Specifications include provisions for testing and sterilization of all new water distribution facilities.
a. ________ Valve cover;
b. ________ Standard cover;
c. ________ Standard hydrant installation.
(n) ________ Street plans, including storm sewers, are complete and conform to the codes applicable thereto and include all of the following:
1. ________ The location of streets and width of pavements conform to those indicated on the approved preliminary plan.
2. ________ Plan shows curb, gutter and sidewalk locations, and include the following information:
a. ________ Corner curb radius is not less than 16'.
b. ________ Curve data for all horizontal curves.
c. ________ Direction of flow along all curbs.
d. ________ No surface water is carried across or around any street intersection, nor for a distance greater than 600'.
3. ________ Cross-sections are submitted as necessary to indicate feasibility of proposed street elevations in relation to adjacent lot elevations and include sidewalk location.
4. ________ Profiles are submitted for all paving centerlines and storm sewers and indicate:
a. ________ Catch basin invert elevations;
b. ________ Minimum pipe size is 12" I.D., except that a lead from a single inlet may be 10" I.D.
c. ________ The grade of each section of sewer shown by percentage in accordance with accepted engineering practice.
d. ________ Storm sewer elevations do not conflict with any other underground utilities.
e. ________ Storm sewer is connected with an adequate outfall.
f. ________ Curve data is given for vertical road curves.
5. ________ The storm sewer system is designed to provide sufficient capacity for the drainage of upland areas contributing to the storm water runoff on the street.
________ Storm sewer design computations are submitted with plans.
6. ________ A surface water drainage pattern is shown for each block.
7. ________ Material specifications comply with standards of the municipality and include:
a. ________ Paving base materials;
b. ________ Paving surface materials;
c. ________ Concrete;
d. ________ Pipe materials.
8. ________ Typical cross-sections and details include the following:
a. ________ Collector street;
b. ________ Minor or cul-de-sac street;
c. ________ Concrete curb and gutter;
d. ________ Concrete sidewalk;
e. ________ Standard manhole;
f. ________ Standard cover;
g. ________ Catch basin;
(o) ________ Street light plans are complete and include the following:
1. ________ Pole locations;
2. ________ Spacing;
3. ________ Average maintained footcandle illumination (calculated).
a. ________ Type of base and pole;
b. ________ Bracket or arm;
c. ________ Luminaire, indicating type of lamp and wattage;
d. ________ Mounting height.
(p) ________ Parkway improvement specifications are complete and include provisions for:
1. ________ Removal of stumps and trees that cannot be saved, boulders and all other similar items.
2. ________ Grading, installation of topsoil and seeding or sodding.
(q) ________ Street signs are shown to be installed at all street intersections not previously marked.
