No permit for cutting and rebuilding of streets, sidewalks or curbs shall be issued until the owner, or, in his absence, the contractor complies with the following provisions:
(a) The applicant shall pay a fee relative to the construction, according to Section 1713.05.
(b) The applicant shall furnish the City an agreement (standard agreement form to be provided by the City) which shall provide for the following:
(1) Approval of the City Fire and Police Departments if, for any reason, it is necessary to block traffic or the street, alley or passageway to complete the work under the permit; and
(2) Liability commitment on the part of the contractor for any and all injuries resulting from the construction and to save harmless the City from any legal action that might occur or arise from negligence on the part of the applicant involved, approved by the City Attorney.
