§ 12-6-3 REQUEST FOR SERVICE.
   A.   Domestic water service within the service limits of the city.
      1.   To obtain domestic water service from an existing distribution main, application should be made at the office of the Building Inspector. The applicant should make sure that the name of the customer, the street address, and the legal description of the property to which service is to be rendered are correctly entered upon the contract or agreement for service. When feasible, the applicant should, before making application for service, determine whether the property to be served is adjacent to the city distribution system.
      2.   A customer first requesting water service for a property is required to pay a service connection fee to defray installation costs. Additional services to the same property may also be obtained at similar fees per meter connection. The service connection fees will be collected only once for any one metered service. Section 12-6-8 describes fees for various size services.
   B.   Fire protection water service within the service areas of the city. To obtain the service, application should be made at the office of the Building Inspector. In case of a new fire line and/or combination fire line and domestic water service installation, it is necessary that the city be provided both plot plans and plumbing plans for a cost estimate. After the cost has been determined, it will be necessary for the applicant to sign the agreement or contract for service or to send a letter authorizing the installation. However, every applicant shall prepay the estimated cost of the installation from the water main to the property line, including an isolating valve, a detection check valve for determining leakage or illegal usage (plus the extension of the proper size main if necessary), street repair, sidewalk repair, labor, concrete vault for detection check valve, other related materials, and any other costs as may be incurred in rendering service.
   C.   Fire hydrant service. A developer or customer requesting the installation of fire hydrants in the service area of the city shall be required to pay the installation cost, including the isolating valve, fire hydrant assembly, pipe, fittings, street repair, sidewalk repair, labor, materials, and any other costs as may be incurred in the installation. No fire hydrants may be installed on any water main of less than six-inch size. A fire hydrant service charge shall be required as described in § 12-6-10.
