§ 118.02 APPLICATION.
   Any person desiring to procure a taxicab operator’s permit shall file an application for such permit with the City Clerk/Tax Assessor on a form requiring the following:
      (1)   The applicant’s full name and place of residence.
      (2)   The applicant’s place of birth.
      (3)   The age, color, height, color of eyes and hair of the applicant.
      (4)   Whether the applicant is a citizen of the United States.
      (5)   The length of time as a resident of the city and each place of residence while here.
      (6)   Each place of residence for the five years last past.
      (7)   The name and address of each employer during the last five years.
      (8)   Whether the applicant is married or single.
      (9)   Whether or not he has ever been convicted of any public offense, and if so when and where, together with the nature of the offense.
      (10)    Whether he has been at any time arrested and charged with the commission of any felony.
      (11)    When and where and for what period of time, if any, he has had experience as a taxicab driver, and if so whether or not his license was ever revoked, and if so for what cause.
   (B)   It shall be the duty of the City Clerk/Tax Assessor or the Police Department to provide all forms and license plates required by this chapter.
   (C)   Each application shall be sworn to by the applicant and have the officer’s jurat as a part of same, and there shall also be attached to the application at least two affidavits of reputable citizens of the city reciting that they have known the applicant for at least two years and that his reputation is good for honesty, industry, sobriety, and good citizenship generally. The applicant shall fill out all of the requirements of the application, and shall file same with the City Clerk/Tax Assessor who, upon approving its form of execution, shall deliver it to the Chief of Police who shall retain same as a part of the permanent records of his office following its being acted upon by the examining board provided for herein. Each application shall be referred by the Chief of Police to the examining board consisting of himself, the City Clerk/Tax Assessor and the chairman of the police committee of the City Council, or someone designated by him.
   (D)   Before any operator’s permit shall be issued the City Clerk/Tax Assessor shall place a notation upon the application in the space provided for therein indicating that the Examining Board has considered the application; when it has considered it; and whether it has approved or rejected it; and below his notation he shall subscribe his name as Clerk of the Examining Board.
