§ 115.16 APPLICATION FOR LICENSE; FEE.
   (A)   Any person who desires to engage in the practice of being a fortune teller in the city shall file an application with the City Clerk/Tax Assessor and deposit with the clerk a fee of fifty ($50) dollars which shall be used by the clerk to defray the cost of notices required to be published in a newspaper, and to pay the clerk’s cost of processing the application.
      (1)   The true name, date of birth and social security number of the applicant.
      (2)   The exact location of the proposed place of activity.
      (3)   The occupation of the applicant for the five (5) years immediately preceding the date on which the application is filed.
      (4)   A description of any prior criminal convictions of the applicant, other than traffic violations, within the last five (5) years and the jurisdiction where the offense was committed.
      (5)   Any prior applications, licenses or permits for the operation of the same or similar business and the jurisdiction where they were submitted, obtained and/or suspended or terminated in the last five (5) years.
      (6)   If the applicant has or intends to have employees or individual contractors also engaged in fortune telling at the place of entertainment, the applicant shall provide the name, date of birth, and social security number of all employees or individual contractors within three (3) business days of their first work day at the place of entertainment.
