§ 119.27 APPLICATION.
   (A)   The applicant for a taxicab driver’s license under the provisions of this subchapter shall fill out upon a blank form to be provided by the Police Department a statement containing the following information.
      (1)   His or her full name, residence, and place of residence for five years previous to the date of his or her application.
      (2)   His or her age, color, height, color of eyes and hair, place of birth, length of time he or she has resided in the city, whether a citizen of the United States, the place of his or her previous employment, and whether married or single.
      (3)   Whether he or she has ever been convicted of a felony and, if convicted, whether he or she has previously been licensed as a taxicab driver, and, if so, when and where.
      (4)   Whether his or her license has ever been revoked and for what cause.
   (B)   This statement shall be signed and sworn to by the applicant and filed with the Police Department as a permanent record.
   (C)   Each applicant for a taxicab driver’s license under the provisions of this subchapter shall file with his or her application two recent photographs of himself or herself of a size which may be easily attached to the license, one of which shall be attached to the license when issued, and the second filed, together with the application, with the Police Department.
