§ 119.17 ISSUANCE; DUPLICATES.
   (A)   If, upon the inspection required by § 119.12, the taxicab is found to be of lawful construction and in proper condition in accordance with the provisions of this chapter and when so licensed, there shall be delivered to the owner, in addition to the license, a card of the size and form prescribed by the Board of Public Works and Safety.
      (1)   The card shall contain the official city license number of the taxicab, together with the date of inspection and a statement to the effect that in case of any complaint the Police Department shall be notified, giving both the state and city license numbers of the vehicle.
      (2)   The card shall be signed by the Clerk of the Board of Public Works and Safety and shall contain blank spaces upon which entry may be made of the date of every inspection of the vehicle by the inspector designated by the Police Department.
      (3)   The license card shall be of a distinctly different color each year and shall contain the name of the person owning the taxicab and the state and city license numbers issued therefor.
   (B)   Upon the payment of the sum of $l to the Clerk-Treasurer and furnishing proof by affidavit of the loss or destruction of the license or such license card, a duplicate shall immediately be issued to the licensee with the notation thereon that it is a duplicate.
