An application for a license to display a mechanical amusement device or juke box must be made to the Mayor upon such forms as shall be prepared therefor by the Mayor. The application must be made by the owner or proprietor of the business or place at which the mechanical amusement device or juke box is to be displayed. The application shall state the name of the owner of the place of business, the address of the place for which the license is applied, the residence address of the owner, the serial number and name of the manufacturer of the mechanical amusement device or juke box, the owner of the mechanical amusement device or juke box, the name and address of the distributor of the mechanical amusement device or juke box, the nature of the business in conjunction with which the mechanical amusement device or juke box is to be displayed and such other further information as may be required by the Clerk-Treasurer.
