733.03 APPLICATION; FEE; EXPIRATION.
   Applicants for permit and license under this chapter must file with the Safety-Service Director an application in writing on a form to be furnished by the Safety-Service Director which shall give the following information:
   (a)   Name and description of the applicant (all agents of the applicant who will offer or sell goods or services within the City must separately apply);
   (b)   Social security number, permanent home address, full local address, phone number and email address, if any, of the applicant;
   (c)   A brief description of the nature of the business and the goods to be sold;
   (d)   If employed, contact information (name, address, phone number and email address, if any);
   (e)   The length of time for which the right to do business is desired provided, however, that this right may not exceed a period of three (3) months per application;
   (f)   Vehicle information (make and year of vehicle; license plate number) that will be used by applicant;
   (g)   Driver’s license number of applicant;
   (h)   A photograph of the applicant, taken within sixty days immediately prior to the date of the filing of the application, showing the head and shoulders of the applicant in a clear and distinguishing manner;
   (i)   The fingerprints of the applicant;
   (j)   A statement as to whether or not the applicant has been convicted of any felony or misdemeanor or violation of any Municipal ordinance, the nature of the offense and the punishment or penalty assessed therefor other than a traffic moving violation; and
   (k)   The City’s income tax form must also be filled out and filed with the application.
   At the time of filing the application, a non-refundable administrative fee of twenty-five dollars ($25.00) shall be paid to the City to cover the cost of investigation of the facts stated therein. An additional non-refundable fee of forty-five dollars ($45.00) is required per applicant for criminal background assessments. All permits issued hereunder shall be for a period of ninety (90) days from the date of issuance.
