915.04 APPLICATION FOR MEMBERSHIP.

   Memberships in and for the use of such municipal recreation facilities shall be made on the application of a parent or guardian, husband or wife, or individual, in which such applicant shall submit all residence requirements and ages of all members of that family desiring to become members. Upon payment of all the aforesaid stated fees for such family, and each member having his photograph taken and identification cards completed, the Recreation Director shall issue their membership thereto.
