1041.04 EXTRAORDINARY WASTE; PERMITS AND EXEMPTIONS.
   (a)   Owners of property classified industrial, as detailed in this chapter, and/or property producing extraordinary waste, shall be required to obtain a permit to discharge into the City sewerage system, unless exempted by the City. The permit and/or exemption shall be issued by the City and shall be predicated upon an evaluation of the premises by the City Engineer, together with information submitted by the applicant. Application forms, permit forms and exemption forms shall be as established by the City and shall include, but not necessarily be limited to, the following information:
      (1)   The name and address of the applicant;
      (2)   The address of the facility generating extraordinary waste;
      (3)   The point of the proposed discharge;
      (4)   The volume of essential characteristics of the proposed discharge;
      (5)   Plans and specifications for building sewers, together with proposed facilities for control, monitoring and gauging, if required; and
      (6)   Any other essential data that may be determined by the City Engineer to be necessary in the determination of the acceptability and suitability of permitting such waste to be discharged into the public system.
   (b)   No person shall discharge, cause to be discharged or attempt to discharge any extraordinary waste to any public sewer in the City until he or she has obtained a proper permit therefor as outlined in this section.
