541.02 LICENSE APPLICATION.

   Every applicant for a license to operate a self-service retail dry cleaning and/or laundry establishment shall file a written application with the Mayor or his authorized representative containing the following information:

   (a)   The name under which the applicant will do business.

   (b)   The address of the establishment or proposed establishment.

   (c)   The full name and address of the owner of the premises, the owner of the business and the operator of the business.

   (d)   The type of business and the number of machines of each type to be used.

   (e)   The name and model of each machine and its capacity.

   (f)   The type of cleaning fluid to be used.

   (g)   A statement that the applicant has or will procure liability insurance coverage in the amounts of one hundred thousand dollars/three hundred thousand dollars ($100,000/$300,000) for personal injury or death and fifty thousand dollars ($50,000) for property damage.

   (h)   Such other information as the Mayor or his authorized representative may require.
