Such application shall state the name of the person, firm, or corporation desiring the permit and the name of the person doing the work; shall recite specifically, and illustrate by sketch or plan on the back thereof, the exact location, width, depth, and length of the excavation desired to be made; and shall state for what purpose it is required, and the probable length of time required for its completion.
