The City Manager may revoke any permit issued under this chapter if he or she determines that a permittee has:
(A) Violated the terms and conditions of his or her permit;
(B) Violated any provision of this chapter;
(C) Failed to factually report the wastewater constituents and characteristics of his or her discharge when required to do so;
(D) Failed to report significant changes in specifications, operations, or wastewater constituents or characteristics;
(E) Refused reasonable access to the users premises for the purpose of inspection or monitoring; or
(F) Failed to pay any fee or sewer service charge required by this chapter.
