§ 15-1-31 PERMIT; APPLICATION; CONTENTS.
   The application for a special event permit shall set forth on a form to be furnished by the Director of Parks and Recreation:
   (A)   The name, address, and telephone number of the person or organization seeking to conduct such special event;
   (B)   If the special event is proposed to be conducted for, on behalf of, or by an organization, the name, address, and telephone number of the headquarters of the organization, and of the authorized and responsible heads of such organization;
   (C)   The name, address, and telephone number of the person who will be the special event chairperson and who will be responsible for its conduct;
   (D)   The date or dates and times of day when the special event is to be conducted;
   (E)   A general description of the activities which will be conducted on all dates applied for; and
   (F)   The proposed location of the special event.
