Each permit shall state the following:
(A) A description of the helicopter;
(B) The name of the owner and operator, the name of another person or association, if any by whom arrangements for an event are being made, and the name of the person who will be personally responsible for conduct of the event;
(C) The flight(s), descent(s), and ascent(s) authorized, including date(s) or period (not to exceed one year), minimum height(s) and route(s) of flight(s), and location(s) of descent(s) and ascent(s); and
(D) Such other information as the City Manager shall determine to be necessary or desirable in order to effect compliance with this article.
