The application for the permit shall set forth:
(A) The name of the organization, the address, and telephone number of the headquarters, if any, of the organization;
(B) The name(s), address(es), and telephone number(s) of the authorized and responsible head(s) of the organization;
(C) The name(s), address(es), and telephone number(s) of the person(s) who will be present and in charge of the event and responsible for its conduct;
(D) The nature of the event;
(E) The approximate number of persons who will participate in the event and of invited guests, if any;
(F) The place of the event;
(G) The date(s) of the event and the hours during which it will be conducted; and
(H) Any additional information which the City Clerk shall find reasonably necessary to enable the City Council to determine whether a permit should be issued.
