Application for such license shall be made to the City Clerk setting forth the name of the applicant, the dates upon which a dance or dances are to be held, a description of the premises upon which said dance or dances will be held, together with the statement of the applicant that he or she will not violate or permit the violation of any law of the state, or any provision of this municipal code upon the licensed premises. The application shall be accompanied by the fee provided in Chapter 6, Article 6 of this code. The license may be for one or more days, for a period of six months, or for a period of one year.
