§ 94.20 APPLICATIONS.
   Any person desiring a permit required by this subchapter shall file an application therefor with the Village Clerk upon a form provided by the Clerk, setting forth the name and address of the applicant, the name of the owner of the device, the date upon which it is intended to be used and other information as may be prescribed.
