(A) Application. Applications for business license shall be made in writing on forms furnished by the Village. Each application shall contain the following information:
(1) Type of ownership of the business (i.e. partnership, sole proprietorship, individual, corporation, etc.);
(2) Affidavit of ownership and right to use business name. Corporations shall file resolution authorizing the applicant to apply for the business license.
(3) List of partners, individuals or corporate officers;
(4) Business telephone number and address (if individual, then also provide residence telephone number and address);
(5) Trade name and trademark under which the public passenger business will be conducted;
(6) Colors and color scheme of the public passenger vehicles operated by the public passenger business;
(7) A list of all public passenger vehicles, showing the manufacturer, make, model, year of manufacture, vehicle identification number (VIN) and state license plate number of each;
(8) Certified copies of all public liability and property damage insurance policies;
(9) Address and telephone number of dispatch office;
(10) Current name, residence address and telephone numbers of every chauffeur or driver. Every licensee shall notify the Village Clerk of any change in the information provided on the application or any other information on file with the Clerk within 30 days of the occurrence of said change.
(B) Renewal. Each business license shall be renewable from year to year, upon filing of an application therefor accompanied by the required fee(s), prior to October 1. If the applicant meets the requirements of this chapter, the Village Clerk shall renew the license for a term ending October 31 the following year.
(C) Issuance. Upon receipt of an initial application and proof that vehicles and equipment have passed the inspections required by this Chapter, the Village Clerk shall issue the business license. Upon receipt of an application for renewal, a recommendation for renewal from the review board or an order authorizing renewal from the Village President, and proof that vehicles and equipment have passed the inspections required by this Chapter, the Village Clerk shall renew the business license.
(D) Appeals. Appeal from the refusal of the Clerk to issue a license under this section shall be taken to the Village President. Such appeal shall be made in writing within five business days of the Clerk's notice of non-renewal. Appeals to the Village President shall be conducted in accord with § 123.24(C).
