Upon the filing of a notice to show cause, under § 12.2710 or § 12.2711, the agency shall serve a copy thereof on the respondent. The agency may include with the notice any information which it deems appropriate. The notice shall include or be accompanied by a statement that the applicant or licensee respondent may request a hearing by filing an answer within 15 days after service upon him or her of the notice to show cause and that the failure to do so will constitute a waiver of his or her right to a hearing and an admission of the matters alleged in the notice to show cause. The answer may be filed by delivering or mailing the answer to the agency.
Service of a notice to show cause or an answer may be made by personal service or by mail.
