§ 12.2710   License Application; Formal Hearing.
   A formal hearing to determine whether a license should be granted, issued or renewed shall be initiated by filing a notice to show cause why such license should be issued. [If the renewal is to be issued merely on the payment of an additional fee, without further showing of entitlement to an existing holder of a license, the proceeding shall be initiated in accordance with § 12.2711.] The notice shall be a written statement specifying the statutes and rules with which the respondent must show compliance by producing proof at the hearing and, in addition, any particular matters which have come to the attention of the initiating party and which would authorize a denial of the agency action sought by the respondent.
