   A notice to show cause issued pursuant to §§ 12.2710 or 12.2711 shall be substantially in the following form:
BEFORE THE BOARD OF SUPERVISORS OF THE COUNTY OF SAN BERNARDINO [or other agency]
In Re   [Name of applicant, licensee or other respondent],   NOTICE TO SHOW CAUSE
   Respondent.
TO:   [Respondent]
You [or: you, and each of you,] are hereby given notice to show cause [why the _________________ license applied for by you should be granted -- or, why the _________________________ license previously issued to you should not be revoked, suspended, etc.: -- or other action should not be taken].
You are further notified that -- [here include factual and legal allegations regarding any charges].
You may file a written answer with the [Board of Supervisors or other agency] at its office located at [address of agency] within fifteen (15) days after service upon you of this Notice to Show Cause. Failure to file an answer will constitute a waiver of your right to a hearing and an admission of the matters set forth in this Notice.
