(A) Any person, firm, corporation, or association displaying for public patronage or keeping for operation any COIN OPERATED AMUSEMENT DEVICE as defined in § 112.15, shall be required to obtain a license from the village, upon payment of a license fee. Application for such license shall be made to the Chief of Police upon a form to be supplied by the Village Clerk for that purpose. Each license shall become effective as of May 1 of each year and expire as of the following April 30.
(B) Each license shall recite the following information:
(1) The name of the person, firm, corporation, or association to whom the license is issued;
(2) The premises upon which the machines or devices are situated;
(3) The number of machines or devices authorized to be placed on such premises pursuant to the terms herein; and
(4) The total license fee paid pursuant to § 112.20.
