5.132.020   Application for and issuance of permit.
   Every person engaged in the art of tattooing or conducting or maintaining a place where tattooing is practiced in the city shall make a written application to the health officer of the city for a permit to conduct such business. Such application shall be signed and sworn to by the applicant and shall state the true or fictitious name of the applicant, his or her true age, present street address and other address where the applicant may have lived or carried on such business during the six months immediately preceding the date of application, the name under which and place where the applicant proposes to conduct such business in the city.
   The application shall also include a photograph of the applicant of a size prescribed by the city health officer. The photograph shall be attached to the original application.
   Upon examination of the application by the city health officer, if he or she shall find that the statements set forth in the application are true and the applicant is of good moral character and proficient in the practice of tattooing, he or she shall issue a permit to the applicant.
