2-5-4: APPLICATION FOR LICENSE:
   A.   Contents Of Application:
      1.   The application for a license shall contain the following information:
         a.   The applicant's name, permanent address and local address, if any.
         b.   A brief description of the nature of the business and the goods to be sold.
         c.   If the applicant is employed, the name and address of his/her employer.
         d.   The names and addresses of the officers or directors of the firm or organization by which the applicant is employed or which he/she is representing.
         e.   The period of time within which the applicant intends to conduct activities, which includes the dates and times of day.
         f.   The source of supply of the goods or property proposed to be sold or, in the case of goods or property to be sold by solicitation of orders, where such goods or products are located at the time the information is submitted to the city clerk-treasurer and the proposed method of delivery.
         g.   If the applicant is a nonprofit, religious, charitable, patriotic or philanthropic organization, a copy of the organization's tax exemption status.
         h.   If the applicant is conducting a temporary sale on another's property, written consent from the person who owns the area to be utilized for the temporary sale.
         i.   A temporary scaled drawing of a temporary sale area, showing all proposed temporary structures, such as tents, booths, buildings, signs and lights, and other details as may be determined by the city.
      2.   If products are to be sold from a motor vehicle, the application shall contain a description of the vehicle, including serial number, license number, make and model, and shall also contain the name of the insurer providing liability coverage on the vehicle.
   B.   Changes In Contents: Every license holder shall promptly submit to the city any changes in the foregoing data. (2005 Code)
