(A) Every application for a license shall be made in writing to the Village Clerk on a form provided for that purpose. The application shall contain the name of the person desiring the license and the place of business of the applicant. If the applicant is a partnership or firm, the application shall contain the names and addresses of each of its members, if a limited partnership, the names and residence addresses of each general partner thereof, and if a corporation, the names and addresses of the principal officers.
(B) The application shall also state the number and description of vehicles which the applicant intends to use or operate in the business, the method of disposal, and the place where the applicant intends to dump and a written authorization to use the facilities by a landfill operator who has the proper EPA permit.
