The lien notice and certificate as provided by § 93.40 shall be in form substantially as follows:
NOTICE OF LIEN
Pursuant to the authority vested by the ordinances of the town, the Town of Rockville, through its agents, did on or about the ______ day of _____________, 20___ cause the premises hereafter described to be rehabilitated, or the building or structure on the property to be repaired or demolished, in order to abate a public nuisance on the real property; and the Town of Rockville did on the ______ day of ____________, 20___, assess the cost of such rehabilitation, repair or demolition upon said real property, and the same has not been paid nor any part thereof; wherefore the town does hereby claim a lien pursuant to Indiana Law in the amount of the assessment, to wit: the sum of $____, and the same shall be a lien on said real property until the same has been paid in full and discharged of record, said real estate being more particularly described as follows:
