§ 152.184 GRADING AND DRAINAGE PERMIT APPLICATION FORMS.

   A Class 1 or Class 2 grading and drainage permit application shall include:

   (A)   Name(s), address(es), and telephone numbers of the owner and developer of the site, the contractor(s), and of any consulting firm retained by the applicant identifying the principal contractor;

   (B)   Certification that all construction covered by the grading and drainage permit will be undertaken in compliance with §§ 152.155 through 152.159;

   (C)   A sketch acceptable to the Zoning and Building Director, or his or her designee, showing the amount of impervious area being created;

   (D)   A sketch acceptable to the Zoning and Building Director, or his or her designee, showing the amount of disturbed ground area being created; and

   (E)   An application fee.
