(A) The owner of a dog or other animal which bites a human being, shall immediately notify the Police Department of the bite and address of the person bitten, if known.
(B) Any person who is bitten by a dog or other animal shall forthwith notify the Police Department of the bite giving a description of the dog or other animal, the time and circumstances of the bite and the name and address of the owner, if known.
(C) When a doctor, veterinarian, hospital employee or other person has information that a person has been bitten by a dog or other animal, the person shall forthwith notify the Police Department.
