Any person desiring a street solicitor's permit shall apply to the Clerk-Treasurer upon a form furnished by the Clerk-Treasurer. On the application form, the applicant shall state his full name, his residence, his age, for whom he is soliciting and one character reference. In addition, he shall furnish such additional information as may be required by the application form.
