Any person requiring a license for any such motor vehicle shall prepare an application upon a form provided by the village, setting forth the name and address of the applicant, a description of the motor vehicle, its state license number, if any, and such other information as may be prescribed. Upon payment by the applicant of the license fee provided in section 9-1-3 of this chapter, the village shall issue the license. (Ord. 2844, 9-21-1999)
