(A) There is hereby established the Healthy Community-OCCF Fund into which funds from the Ohio County Community Foundation may be deposited and utilized for the dental health of the citizens of Ohio County, Indiana that are in need of assistance and as outlined in this section.
(B) The funding for the Healthy Community-OCCF Fund shall be provided by the Ohio County Community Foundation and deposited into the Fund.
(C) Those seeking assistance for dental health needs shall apply with the Ohio County Community Foundation and the Foundation shall submit for payment those applications which qualify for assistance under the guidelines of the Foundation. Payment of claims must be approved by the Common Council of the City of Rising Sun, Indiana.
(D) Funding is only available to residents of Ohio County, Indiana and for dental health expenses incurred at dentist and/or orthodontist office(s) located in Ohio County, Indiana.
