(a) A Notice of Final Determination of a liability for a violation of Section 456.03(a) of this Chapter shall be issued if the vehicle owner fails to pay the fine specified in the Notice of Violation or request a hearing by the response date specified in the Notice of Violation.
(b) The Notice of Final Determination shall be mailed to the registered owner. The Notice shall state that the registered owner has exhausted all available means of challenging his/her liability for the violation of Section 456.03(a) of this Chapter, and that the registered owner is liable for paying a one hundred dollar ($100.00) fine, along with a one hundred dollar ($ 100.00) late payment and that the one hundred dollar ($100.00) fine and the one hundred dollar ($100.00) late payment fee is a debt to the Village. The amount of the fine and the late payment fee specified in the Notice of Final Determination shall be two hundred dollars ($200.00) or such lessor amount as may be determined by the Hearing Officer.
(d) The Notice of Final Determination shall contain a statement that a failure to pay any fine and late payment fee that may be due and owing to the Village may result in the Village petitioning the Circuit Court of Cook County to enter a judgment against the registered owner for the amount of the fine and any applicable late payment fee and may also result in the suspension of the registered owner's driver's license if the registered owner has accumulated five or more unpaid fines for violation of Section 456.03(a) of this Chapter.
