1024.01 EXCAVATION PERMITS.
   (a)   Permit Required. No person shall tear up or open the pavement or curbing on any street, alley or public ground or otherwise make an excavation in any street, alley or public ground in the Village without first obtaining a permit therefor from the Village Manager, after approval by the Village Engineer.
   (b)   Applications. The application shall state dimensions, exact locations and any other information that the Engineer may require, and shall be made on a form to be supplied by the Engineer.
   (c)   Issuance. The permit shall be granted upon compliance with the applicable provisions of this chapter.

   (d)   Limitation of Location and Dimensions. The permit shall in no case be taken to cover an excavation at any other point or of a dimension greater than six inches either way than that shown on the permit. If it is deemed necessary to make a greater excavation, a new permit is required.
   (e)   Time Limits. No excavation permit shall be issued more than five days before the excavation is to be made or work thereon is commenced.
   (f)   Use of Other Name to Obtain Permit. No person shall allow his or her name to be used to obtain a permit for any other person.
   (g)   Use of Excavation by Two Persons. No two persons shall use the same excavation except by written permission of the Village Engineer.
