No person is eligible for an SOE manager, server or entertainer license, nor shall such a license be issued to an applicant, if one or more of the following conditions exist:
(a) The employer for whom the applicant intends to work does not have or is ineligible to receive an SOE business license for any of the reasons stated in this chapter;
(b) The applicant has been convicted of or plead guilty or nolo contendere to any of the crimes set forth in Section 864.06 during the time period set forth in such section;
(c) The applicant failed to provide all of the information required on the application;
(d) The applicant gave materially false, fraudulent or untruthful information on the application;
(e) The applicant has had an SOE manager, server or entertainer license revoked or suspended in this or any other municipality, county or state during the past five years.
