(a) An application for the license required in Section 858.02 shall be made upon forms furnished by the Village Clerk.
(b) The application shall set forth the following:
(1) The name under which, and the place where, the parking lot is to be operated;
(2) Whether the applicant is an individual, partnership or corporation. If an individual, the name and business and residence addresses of the applicant shall be given; if a partnership, the name, and business and residence addresses of each partner shall be given; and if a corporation, the following shall be furnished: the name, date and state under which such corporation was organized, the amount and value of the capital stock issued by such corporation, the name and business and residence addresses of the officers and managers in charge and the directors thereof and a certified copy of the articles of incorporation.
(3) Whether the premises are owned or leased by the applicant, and, if leased, the name, and residence and business addresses of each owner or part owner thereof;
(4) The number of motor vehicles which may, at any one time, be stored upon the premises;
(5) The hours during which motor vehicles may be stored;
(6) A complete schedule of the rates to be charged for storing motor vehicles and the hours at which changes in rate take place; and
(7) Such other information as the Village may deem advisable.
(c) The application shall be signed and verified under oath by the applicant, if an individual, or by a duly authorized agent, if a partnership or corporation.
