690.02 TREATMENT AND PERMITS.
   (a)   Required. No person shall discharge or deposit any sewage or other waste which creates a public nuisance, a menace to the public safety, pollution or contamination of underground or surface waters or impairs the use of any private sewer, storm drain channel or public or private property, without treating the waste as required by law and without obtaining a permit therefor from the Village Engineer subject to prior approval from the Board of Trustees and its President.

   (b)   Permit Applications. The application for such permit shall be in writing, signed by the applicant and filed with the Engineer, and shall state the following:
      (1)   The name and address of the applicant;
      (2)   The type of waste or sewage to be discharged;
      (3)   The quantity of waste or sewage to be discharged, expressed in maximum amounts per hour per day;
      (4)   The treatment given to discharge, if any; and
      (5)   Proof, by affidavit, of compliance with all Village, County, State and Federal laws concerning pollution of watercourses.

   (c)   Pre-Issuance Investigation and Inspection. The Engineer or his or her designee shall:
      (1)   Cause an investigation to be made of the facts stated in the application; and
      (2)   Cause an inspection to be made of the premises and facilities to be used for the discharges, to ascertain the fitness of such premises and facilities and determine whether or not they comply with the Village, County, State and Federal laws concerning water pollution.

   (d)   Periodic Inspections of Premises and Facilities. The Engineer or his or her designee shall periodically inspect the premises and facilities of any permittee under this section to ensure compliance with all Village, County, State and Federal laws concerning water pollution.

   (e)   Inspection Fees. The fees to be charged for the inspections required in this section are as follows:
      (1)   Five dollars ($5.00) for the initial inspection; and
      (2)   Three dollars ($3.00) for any subsequent inspection.

   (f)   Permit Issuance. Upon completion of the requirements of subsections (b), (c) and (e) hereof, the Engineer may grant a discharge permit to the applicant.
