No permit shall be issued by the Building Department for doing or performing any of the acts or work referred to in Section 901.01
unless and until the Applicant therefor shall have complied with the following requirements:
(a) Application. Complete and file an appropriate application with the City Building Department for permission to do or perform the work or act referred to in Section 901.01
, which in addition to such other information as the City Engineer and/or Building Inspector may request, shall contain:
(1) The name, address, telephone number, fax number, and cell phone number of the Applicant;
(2) The name, telephone number, fax number, and cell phone number of the owner of the area of construction activity;
(3) The area or location where the work or acts are to be done or performed;
(4) A description of the work to be done or acts performed;
(5) An estimate of the number of days to complete the construction activity; and
(6) An estimate of the number of days that the street, sidewalk or right-of-way will be disturbed.
(b) Insurance Policy.
(1) Single Dwelling Residence. In the event the Applicant is the homeowner and the subject property is a single family dwelling, then the Applicant must show proof of homeowners insurance in the amount of one hundred thousand dollars ($100,000) for each person and three thousand dollars ($300,000) for each accident.
(2) Others. Before any permit is issued, the Applicant shall also submit evidence that he or she has obtained and has in effect comprehensive liability insurance, issued by a company authorized to write insurance in the State of Ohio, designating the City of Reminderville as the insured therein, by the terms of which the City is saved harmless from any and all claims for bodily injury, death and property damage within the limits of three hundred thousand dollars ($300,000) for each person and five hundred thousand dollars ($500,000) with one million dollars ($1,000,000) umbrella coverage for each accident arising out of construction activity done or performed, or any work done or acts performed incident thereto, pursuant to the issuance of the road opening permit.
