No permit shall be issued, save and except upon the filing by the applicant therefor with the city clerk of the city a verified application addressed to the city council of the city setting forth the name, age and place of residence of applicant, and the kind and number of motor vehicle to be used pursuant to such permit, and a statement that the applicant will comply with all the provisions of this chapter.
