§ 95.30  APPLICATION.
   Written application for a license shall be made annually by the owner on a form provided by the city. The owner shall be identified by name, address and telephone number and the dog or cat sex, age, breed, color and call name and the rabies inoculation certificate number, date and administering veterinarian given.
